Job Placement Form

Thank you for taking the time to provide us with information about your job placement. One of the federal requirements of our program is that we gather placement information and meet minimum placement percentages. The standards are high, and we have to show results to meet those standards.  Your responses help to ensure that we can continue providing this valuable training.  Statistics will be reported on a group basis, so feel secure that your privacy will be maintained and respected.

REMEMBER:  Many types of working situations qualify as a placement. If you are self-employed, employed as a consultant (1099 employment), "temping" through an agency, or working one or more part-time jobs, we can use this information.
