Reschedule Request Form

Full Circle Computing is pleased to offer this reschedule request form to you to make scheduling changes easier and more efficient.

• Complete this form indicating the course or courses you need to change and submit.
• Consider us notified that you can’t make the date, and cross off the course or courses that you can’t attend from your schedule.
• We will make the appropriate changes and email a revised schedule to you.  Please allow 7 – 10 days for a revised schedule.

When completing this form, please let us know if there are any dates that you are not available for training.  That will assist us in selecting alternate dates for your class.  We will email your revised schedule if we have your email address on file.  If we do not have an email address for you, we will print your schedule and provide you a hard copy.

PLEASE NOTE: Date change requests less than one week in advance may result in your class being rescheduled on a retake (space available) basis.  Please note that excessive reschedule requests will affect the effectiveness and length of your training program.
